Elevated ICP

Eyes directed forward

Axial image, consider both longitudinal and transverse images

Locate optic nerve, measure diameter of optic nerve sheath 3 mm posterior to the globe

2-3 measurements taken

Upper limit: 4 infants, 4.5 mm children, 5 mm adults

*note bilateral optic nerve sheath dilation in elevated ICP, if unilateral other etiology must be considered (i.e., tumor obstructing fluid dynamics in affected eye)

Diffuse swelling, widened orbital soft tissue, thickened medial rectus muscle, fluids in sub-Tenon’s space deep to the sclera
